You can use this service if you:
- want to register at the surgery
- live within our catchment area
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
- details of previous GP surgery
- basic health and medical information
- provide the surgery with photographic ID and proof of address or a copy of a birth certificate for a child after registering
